The objectives of this presentation are:

  1. Review the different groups of beta-lactam agents including the penicillins, cephalosporins, carbapenems, and monobactams.
  2. Review the different mechanisms of beta-lactam resistance in gram-negative and gram-positive bacteria.
  3. Present the Ambler classification of beta-lactamases and review the substrate spectrum of each enzyme. We will describe in detail the penicillinases, the broad spectrum beta-lactamases, the extended-spectrum beta-lactmases, the AmpC beta-lactamases, the OXA beta lactamases, and finally the metallo beta-lactamses.
  4. Review the available beta-lactamase inhibitors including the newer agents such as avibactam, rebelbactam, and vaborbactam.
  5. Identify optimal antibiotic regimens for common infectious agents in the ICU setting, more specifically, Carbapenem-resistant Enterobacteriaceae (CRE), multi-drug resistant pseudomonas, and MDR Acinetobacter.
